The procedure aims to improve the quality of life of people with disabilities, adults at risk, the over 65-year-olds unable to self-handle living in the LAG Lom, by providing affordable and high-quality social and health services in the community quality and responsive to their needs. Results to be achieved through the implementation of the procedure: Opportunity to implement preventive actions aimed at the elderly and their social and health care lems; Opportunity and improved access to services for 100 persons aged over 65, people with disabilities and adults at risk and unable to organize their own lifestyle and day-to-day activities; Overcome the lack of social contacts for people over 65, people with disabilities and adults at risk; Provided in the home environment social and health services; Two social service providers have extended their scope of service and provided social inclusion services.
