The procedure will support the implementation of measures to promote the inclusion of the unemployed and inactive in the labor force and labor market participation, with a special additional focus on certain disadvantaged groups.
The main goal of the measure is to improve the access of people with disabilities and people over 65 who are unable to self-care to social inclusion and health care services. Support for the development of long-term care will play a preventive role and prevent placement in specialized institutions and unnecessary hospitalization, and will contribute to changing the current institutional and still medically oriented model of care for the elderly and people with disabilities. As a result of the implementation of the measures under this specific goal, the quality of life of the people in need of constant service in their daily life and of the persons with disabilities will be improved by providing long-term care, incl. supporting and integrated social and health services in the community and at home. Services should be geared to the consumer himself and to his changing social and health needs.
